INSTRUCTIONS: • Do not include your face in photos (unless procedure involves face) • Hair should be tied back, if possible • Hang arms by your side with your hands approximately 12 inches away from your body • Posture should be identical in all 3 photos • Take photos in front of a neutral background • Take photos in a well-lit environment, use flash if necessary • Photos should be sent in color and in JPG format
